- First, verify your identity by providing your Date of Birth and Employee ID.
- Second, follow the link to complete the Personal Health Survey. The survey can be complete on most computers, tablets and mobile devices with internet access.
- Third, after completing the Personal Health Survey, schedule your on-site screening by clicking the available Screening Events shown at the end of the survey.
